If you send a message through the contact form, the name, email address, phone number and message you enter are emailed to our contact inbox so we can reply. The website itself does not keep a copy.

Do not send health information here

The contact form is not a secure channel for health information. Please do not include diagnoses, medications, or other health details. To share clinical information, use the secure client portal or call us.
