Create your account
Sign up with your work email and confirm it from the message we send. This keeps your application tied to you.

Tell us about your practice
Your provider type, solo or group, where and how you see clients, the insurance panels you are on, whether you offer self-pay, your NPI and the specialties and populations you serve. We check NPI numbers against the national registry as you enter them.

Book your onboarding call
A short call with us to answer questions and walk through the Care Coordination Agreement.

Sign the agreement
Each practice signs for itself and keeps its own name, contracts, clients and billing. We recommend having your own counsel review it.

Join the directory
We set up your portal account, your practice appears in the member directory, and it is added to the next dated network roster.
